Funds raised from this event will help fund our transition backpack program, helping brain injury survivors discharging from a hospital or rehab facility back to the community.
As one of the first point of contacts for patients, we witness firsthand the frustration that brain injury survivors and caregivers can experience due to lack of resources available for individual recovery and transitioning back into the community after an inpatient rehab or hospital stay.
Once patients are discharged they lose their 24/7 support and are typically sent home with a family member or caregiver with little medical or no medical training. Frequently, the caregiver will state they feel completely overwhelmed when going home. When a problem arises, they are unsure of how to handle the situation or even who to call for help. The transition backpack transition program will offer the caregiver and the patient essential and useful information for these situations and will be a lifeline to help answer ongoing questions.
In addition, when patients receive the Backpack, they can also opt-in for a 12 month Neuro Resource Facilitation program through BIAOH. This is a wonderful opportunity for the survivor to work directly with a Resource Facilitator/Advocate who will contact the TBI survivor on a regular basis and help address ongoing challenges such as how to apply for disability services, how to address financial needs (food, utilities, insurance), how to get legal assistance, etc. This will allow the individual to receive person centered supports for their specific needs.
Each backpack contains Information such as how to access the BIAOH toll-free Helpline, an informational family guide, an ice pack, ear plugs, a sleep mask, a meditation CD, and a water bottle. All of these resources are important tools to help the survivor and the caregivers feel supported in their journey to recovery.
